OTTAWA -- Investigators say broken bilge pumps and lax safety measures
were to blame for an accident on the Ottawa River two years ago that
claimed four lives.

The Transportation Safety Board says that only two of the six bilge pumps
were working on board the amphibious vessel, the Lady Duck. The final
report on the tragedy also blames poor safety practices aboard the craft.

A mother, her two children and a nun were killed in June 2002 after the
Lady Duck sank in a matter of minutes in the Ottawa river.

The craft - a converted Ford truck that could tour on water or land - had
been ordered out of service a year earlier.

There were no injuries in that case.

Transport Canada had inspected the craft and allowed the tour boat to
return to service.
